Membership Terms & Conditions
Membership Duration
All memberships are for a 12-month period from the date of enrollment. Memberships automatically renew unless cancelled with 30 days written notice.
Payment Terms
Annual membership fees are due in full at the time of enrollment. We accept cash, check, and major credit cards. No refunds after 30 days from enrollment.
Service Limitations
Membership benefits apply only to the enrolled member. Services must be provided at Renew Dental Studio. Emergency services subject to availability.
Cancellation Policy
Members may cancel with 30 days written notice. Unused benefits are forfeited upon cancellation. No prorated refunds after the initial 30-day period.
Plan Changes
Plan upgrades are allowed at any time with payment of the difference. Downgrades take effect at the next renewal period.
